Does Your Child Have Native American Heritage?

If your answer is yes, please print and fill out the 506 form below and return it to the main office at your child’s school. By filling out this form you are supporting the Native American Education Program for your student and other students in the Eatonville School District. You do not need to be registered with your tribe or be a certain blood quantum. You do need to know the name of the tribe you have heritage in.
