Sports Registration & Guidelines

Welcome to Eatonville High School Athletics, we are excited you are looking into joining a sports team this year.

On the following pages, you will find information for you that will need to be completed in order to participate in athletics for the school. Please follow the process listed below to get enrolled in sports.

Please keep in mind that students participating in more than one sport during the year must submit the individual safety form for each sport in which he/she participates. However, all other forms only need to be submitted once. Also, please make sure that all forms are completed and signed by all necessary parties.

Incomplete paperwork may delay the athlete from being able to practice, and possibly their participation in one or more games at the beginning of the respective season.

The following steps will need to be taken for the athlete to participate in a sport.

Step #1

Physical Fitness Examination


You will need to ensure that your student's physical fitness examination is current (they are good for two years) and that it will not expire during the sports season.

Please download and print off this form for your doctor, or visit our athletic office at the High School for a copy.

Once you have completed the physical, please return the form back to the athletic office.

Step #2

Skyward Family Access Sports Registration

For a student to sign up for sports on Skyward, both the student and the parent will have to log in to their own separate accounts and fill out the forms. There is a form for Students and a form for Parents. http://family.eatonville.wa-k12.net/


Student Access:

  1. Log into Skyward.
  2. On the left hand side, click Online Forms.
  3. Select EHS Sports for the current year. (Fill Out Form)
  4. Step 1 is Sports Participation (Required) 2 signatures and a date. At the bottom click “Complete Step 1 and move to Step 2.”
  5. Step 2 is the first Sport offered for that season, if this is NOT the sport you will be playing, there is a check box at the top left that says “I do not wish to fill out this optional form.”
  6. On the right hand side you can see all the sports offered. If the last sport on the list is your sport, you will still need to check the “I do not wish to fill out this form” box for each of the other sports.
  7. Step 7 is confirming you have filled out all of the steps and then click the “Submit” button.

Family Access:

  1. Log into Skyward.
  2. On the left hand side, click Online Forms.
  3. Select EHS Sports for the current year. (Fill Out Form)
  4. Step 1 is Sports Participation (Required) 2 signatures and a date. At the bottom click “Complete Step 1 and move to Step 2.”
  5. Step 2 is the first Sport offered for that season, if this is NOT the sport your student will be playing, there is a check box at the top left that says “I do not wish to fill out this optional form.”
  6. On the right hand side you can see all the sports offered. The sport your student is participating in will require you to fill out all of the medical information and emergency contact information. If the last sport on the list is your sport, you will still need to check the “I do not wish to fill out this form” box for each of the other sports.
  7. Step 7 is confirming you have filled out all of the steps and then click the “Submit” button.

Payments can be made through Family Access as well. $50 - ASB (1 per year) $35 - User Fee (1 per season). This will take you to the InTouch Online payments, where you will use your same credentials to log in.

Students will need a current physical on file in the Athletic Office. Physicals are valid for 2 years from the date of the exam. They will also need to be passing all classes.

Step #3

ASB & User Fees

All fines and fees can be paid at the Eatonville High School main office. You can make payments online from your InTouch Online Payments account or in the Eatonville High School's main office. You can also get to the link through Skyward Family Access.

The following payments must be made in order to participate in athletic events:


  • All previous fines must be paid
  • ASB Card - $50
  • Athletic User Fee of $35 per sport
    • Student maximum per year of $70.00.
    • Family maximum per year of $210.

Step #4

Grade Check

Grade Checks
Student Athletes are required to pass all classes. A grade check is done at the beginning of the season and every two weeks after. If a student fails a class they are placed on the following steps:

  • Step 1: Probation, two weeks to get grade up.
  • Step 2: Can go to practice, no contests or traveling until next grade check. Has two weeks to get grades up.
  • Step 3: Removal from team.
Academic Requirements

To participate in an activity or sport representing Eatonville High School in a performance or in competition, a student must pass all classes. Students must be continuously enrolled in school and taking at least five (5) classes (or the equivalent college level credits). All students are subject to grade checks every four and a half weeks.

  1. High School eligibility will be checked at the beginning of each season and at every regular reporting (semester, quarter, midterm) of student grades.
      • Students, who fail to meet the Eatonville School District grade requirement of passing 7 out of 7 classes, will be placed on probationary eligibility for two (2) weeks.
      • If a student’s grades have not improved after two (2) weeks, the student will be declared ineligible to participate in interscholastic contests.
      • A student may be placed on probationary eligibility only once per season.
      • The second time a student fails to meet the grade requirement in a season will result in the student being declared ineligible. The ineligible student will retain this status until they meet the grade requirement.
      • The student must initiate a grade check to become eligible again.
      • Ineligible students may still practice but cannot travel with the team if classes will be missed.
      • Each head coach will determine how long an ineligible athlete may remain on the team.

  2. A student who fails to meet the WIAA grade requirements at the end of a semester, WIAA minimum academic eligibility requirement of passing 6 out of 7 classes, shall be placed on WIAA suspension.
    1. The student shall be ineligible during the suspension period.
    2. The WIAA mandates the suspension period for high school students shall be from the end of the Spring semester through the last Saturday of September in the fall and from the end of the Fall semester through the first five (5) weeks of the Spring semester.
    3. If, at the end of the suspension period, the student is passing the classes required above, the student may then be reinstated for interscholastic competition.
    4. Each student is eligible on Monday of the week following the end of the suspension period
